Policy Statement: The Arizona Address Confidentiality Program (ACP) is a program that helps victims of domestic violence, sexual offenses, or stalking from being located by the perpetrator through public records. The program provides a substitute address and confidential mail forwarding services to individuals and families across Arizona. Participants enrolled in the program use the substitute address instead of their real address. The substitute address is to be used instead of a persons home, work or school address. State and local government agencies must accept the substitute address issued by the ACP - Secretary of State’s office. The ACP accepts all First Class, registered, certified and election mail for the participants and forwards to her/his real address at no cost.

  1. ACP Eligibility: All of the eligibility criteria must be met to apply to the program. The eligibility criteria for applicants are:

    • Is a victim of domestic violence, sexual offense (such as rape, sexual assault, molestation), or stalking;

    • Is in fear of her or his safety;

    • Has moved in the past 90 days to a place unknown by the perpetrator or is planning to move to a place unknown to the perpetrator in the near future within Arizona;

    • Has documentation to support victimization; and

    • Is an adult, or a parent or guardian acting on behalf of a minor or incapacitated person.

  2. Department Responsibilities: When an Address Confidentiality Program (ACP) participant provides an unexpired ACP Authorization Card, participation in ACP is verified and special keying is required with AZCCATS. When the ACP enrollment is not verified the participant is not considered an ACP participant and no special keying in AZCCATS is to be completed. Once the ACP enrollment is verified, key the following in AZCCATS:

    • 1901 W Madison St for the street address

    • The apartment number displayed on the ACP authorization card as the apartment or unit number

    NOTE: The apartment number is the participant’s ACP identification number and must be keyed

    • Phoenix, Arizona 85009 as the City, State, and Zip Code

    NOTE: Do not ask the participant for a physical address or phone number. Do not key the actual residential address or the participant’s phone number into AZCCATS

    Once the ACP participant’s address is keyed, complete the following:

    When available use the collateral contact to verify all of the following eligibility factors:

    • Household composition

    When the ACP participant reports a new residential or mailing address, or they are no longer participating in ACP, document the details to the case file. Any person who intentionally obtains or discloses information regarding the physical address of an ACP participant is potentially guilty of a class one misdemeanor.
